Please fill out the following information for each user receiving a device:
End User Name
Phone Number (existing line or new line)
Current Carrier (can just do one line if all on same)
Choose the device you would like to deploy from drop down menu
Please select:
Storage option
Color preference
Case preference
Screen protector preference
please include:
Trade-in device information
Some fields in the form are completed by Seamless Mobile Services after the request is received.
These include:
Temporary Number
New IMEI
Port Out PIN
New Account Number
Customers do not need to complete these sections. If you dont have some info thats ok.
